Developmental Foundations, Inc. practices person-centered care, which means treating patients as individuals and as equal partners. This model of care is not a medical model; rather, it is a multidisciplinary approach that recognizes that a person may need more than one professional to support them.

DFI, initially and then at a minimum of annually thereafter or as needed, participates in an interdisciplinary team meeting (IDT) that seeks to assess and develop supports in accordance with each individual’s capabilities and potential to manage and improve their own health, personal desires, and goals for the future. We understand that most people want to help themselves, therefore, we assist, through the person-centered approach, to tailor our supports so that we provide a pathway for the acquisition of knowledge, skills and confidence to do so. For individuals that have limited capacity to assume greater responsibility for their care, we work closely with Individual Service Coordinators (ISCs) to ensure relatives, guardians, friends, loved ones, and other providers, are fully involved in the person-centered care planning process.

DFI employs and contracts with a team of professionals to meet the needs of individuals that reside in our CILA homes. This includes, but is not limited to: 

Authorized Disability Service Providers (ADSPs) 

Board Certified Behavior Analysts (BCBAs) 

Licensed Clinical Social Workers (LCSWs) 

Qualified Intellectually Disabilities Professionals (QIDPs) 

Registered Nurses (RNs) 

In addition to the team above, we coordinate and support individuals, as needed, with appointments and referrals to physicians and nurse practitioners, including but not limited to adult medicine, psychiatry, neurology, and other specialists as needed.

 DFI and its CILA homes are supported by Health Services Consultants Inc. 

HSC Inc, an AG Ryle Company is a management and consultation company that ensures individuals, guardians their families and supports receive assistance with the paperwork and processes required for the services they are entitled to under their eligibility for CILA.  

HSC and DFI work together to capture funding necessary for effective service delivery. The DFI homes reap the benefit of staff dedicated to maintenance, upkeep and decorating the homes. Nutrition and health are supported by diets and menus provided by HSC programs. All support, including management of Individual Funds, follows the required federal, state and local safeguards and regulations. With its commitment to encourage staff growth and stability, DFI partners with HSC to insure initial and ongoing training to the employees of the DFI CILA’s. 

DFI and HSC commit to provide training, guidance and monitoring needed for staff provision of excellence in care. HSC offers additional support such as understanding of employee benefits and resources to enhance staff experience. HSC Inc provides Consultation and Management services to the DFI CILA homes and Long Term Care Facilities across the state of Illinois.
